Heartburn reflux and GERD

Heartburn is that uncomfortable, inhibiting digestion-like, you feel burning, sometimes after eating, sometimes at night, which can be felt in the abdomen and chest. It does not have the heart “alarm” symptoms such as pain associated with exertion, radiates to the shoulder, arm or jaw, shortness of breath, or feeling that an elephant sitting on your chest. It actually has literally burned to the esophagus with stomach acid to do. This is a very unpleasant situation that tends to over and over again, especially with the late night or irregular eating habits. If left untreated it tends to get worse, and complications can range from mild to very serious.

Let the discussion on the regular interaction of the esophagus with the stomach. It is a muscular ring at the bottom of the esophagus called the upper gastrointestinal sphincter. It opens to admit food and fluids in the stomach and closes to prevent them from back in the esophagus. That’s good, because the esophagus is not the hard, acid-resistant lining that the stomach does not. In fact, if acid into the esophagus, it literally begins to digest the food to which a condition called erosive esophagitis. There are things that can affect the upper gastrointestinal sphincter. One is a neuropathy, nerve damage from diabetes. Another reason is hiatal hernia, where the sphincter in the breast and does not function. Sometimes it is not so good with older age. Sometimes the lower stomach sphincter at the outlet of the stomach are not open normally, as with a duodenal ulcer, and this leads to an increase in pressure in the upper gastrointestinal sphincter. When acid stomach contents back up into the esophagus, it is called reflux, so the condition often referred to as reflux esophagitis.

Now our attention turns to the production of this acid in the stomach. The parietal cells of gastric acid produced by specialized structures called cannuliculae. The actual biochemical process called the proton pump mechanism. It releases hydrogen protons in its form the gastric juice to a pH value between 1 and 2 for children and 3-5 acidify in adults. The acid is neutralized later in the small intestine. Normal body temperature is slightly alkaline pH of 7.4, so it is no wonder that normal tissue is damaged by the secreted acid.

He has been invited to the White House numerous times, has been named Physician of the Year, as seen in the Wall Street Journal, and has served as Honorary Co-Chairman on the Congressional Physicians Advisory Board
